About This Project
Empowering community-based organizations to better serve and support family and friend caregivers.
The Enhancing Caring Communities Toolkit was developed to assist community-based organizations in improving services and support for family and friend (unpaid) caregivers. The toolkit was created with valuable input from 46 professionals representing 23 home- and community-based organizations across 15 states, all of which participated in the National Caregiving Learning Collaborative. Divided into five key steps, the toolkit includes interactive organizational assessments and additional resources to guide organizations through the process. For each step, templates and examples from participating organizations are provided to demonstrate how to apply the toolkit in real-world settings.
Aims
AIM 1: Equip community-based organizations with tools and resources to assess and improve their caregiver services and support programs.
AIM 2: Guide organizations through identifying service gaps, selecting appropriate caregiver support programs, and planning and implementing those programs effectively.
AIM 3: Foster ongoing evaluation and adaptation of caregiver support programs to ensure their continued success.
The steps outlined in the “Enhancing Caring Communities” toolkit:
Step 1: Identify how your organization currently serves and supports caregivers.
- This step involves assessing your organization’s current services and support structures for family caregivers.
Step 2: Look for a service gap and consider ways to fill it.
- Identify any gaps in the existing services and explore potential ways to fill those gaps to better support caregivers.
Step 3: Choose a program to support caregivers based on the gap you identified.
- Select a specific program or service that addresses the identified gap in caregiving support.
Step 4: Plan and pilot the program you have chosen.
- Plan the logistics of implementing the chosen program and begin a pilot phase to test its effectiveness.
Step 5: Implement and evaluate your program.
- Roll out the program fully and evaluate its effectiveness through feedback and continuous assessment to ensure it meets caregivers’ needs.
These five steps are designed to guide community-based organizations in improving services for family caregivers, ensuring that the programs are both effective and sustainable.
